Physical
examinations are required for participation in interscholastics by the
bylaws of the Arizona Interscholastics Association. You'll need two
forms completed and on file before attending your first practice. One
is the basic
physical (to be completed and signed by your physician)
and the other is a checklist
of potential problem areas (to be completed and
signed by a parent or guardian).
If you're
going on the bus to an away meet, you must have a Travel
permission form on file with the coach. If parents will
be driving an athlete to or from an event, the Parental
tranportation permission form must be on file.
Sometimes
you may need the TUSD
Physician Release for Wrestler to Participate
with Skin Lesion (this proves that cuts and scrapes aren't ringworm
or boils).
And we must
have a TUSD Photo Release form
on file before we can post images of any wrestler.
